1. Copy of Official Birth Certificate issued by county of birth.
2. Photo - Face no larger than 1 inch with child’s full name printed on back (No glasses or hats)
3. Fees –Debit, Visa, MasterCard, American Express or Discover, checks or money orders payable to MECSC, with child’s name in memo Section.
